Your privacy, stated plainly.
Short version: this website does not collect health information, and it is not where your pharmacy records live.
What this website collects
The information you submit through our contact and request forms may include:
- Your name
- Your phone number
- Your email address, which is optional on some forms and required on others
- A preferred call-back time and preferred language
- Your optional consent to receive text messages about your request
- The topic of your request and any general message or notes you provide
- For provider practice-setup requests: the prescriber's name, practice name, NPI, optional DEA number, practice address, and office contact details
What this website deliberately does not collect
No medication names. No conditions, symptoms or diagnoses. No dates of birth, insurance numbers or prescription numbers. No intake answers.
That is a design decision. Your name next to the name of a medication is protected health information, and this website is not built to hold it. When you need to tell us something clinical, we ask you to do it on the phone, where it belongs.
The telehealth intake is not on this website
If you start a telehealth visit, you leave this site and complete an intake operated by TNC Walk-In Clinic, a separate business. Their forms, their privacy practices and their payment processing. You will see a page telling you this before you go. Peak does not receive your intake answers.
How we use what you give us
To respond to your inquiry, arrange a call back, provide requested pharmacy resources, or review a provider practice-setup request and contact the office about it. We do not sell it, rent it, or share it with advertisers or data brokers.
Text-message consent
Where offered, the optional checkbox lets you agree to text-message follow-up from our pharmacy team about your request. Your choice is included with the request sent to our team. The website does not send text messages, and this checkbox does not subscribe you to marketing messages. You can submit the form without agreeing.
Cookies and analytics
This site stores nothing in your browser. No cookies, no local storage, and no tracking of any kind.
Your pharmacy records are a separate matter
Records held by the pharmacy about your prescriptions are governed by HIPAA and by Florida pharmacy law, and by the pharmacy's Notice of Privacy Practices rather than by this page. Ask us for a copy at the counter or by phone.
Children
This website is not directed at children and we do not knowingly collect information from anyone under 13.
